The scaffold was partially dismantled and reassembled several times during the work shift, but was not inspected after each modification. The scaffold was missing cross braces, toe boards, and fall protection. These conditions exposed the employees to struck by hazards and to the hazard of falling approximately 13 feet to the ground below.
