29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  At this job site, employees were working on a tubular frame scaffold where the platform was not fully planked.  The employees were exposed to a fall hazard up to 17 feet to the ground below.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  At this job site, a tubular frame scaffold was erected without base plates affecting the scaffold's structural integrity and exposing the employees to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  At a commercial building under renovation, employees were using a tubular frame scaffold with no access provided by the employer.  Employees were exposed to fall hazards of 17 feet or more to the ground below.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  At this commercial building project, employees were applying stucco to the north side of the building under renovation and did not have fall protection to prevent a fall to ground level.  At this job site personal fall arrest equipment was not in use; neither guardrails nor nets were installed.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:Note:  This requirement does not apply to float (ship) scaffolds where an edging of 3/4 inch x 1 1/2 inch (2 x 4 cm) wood or equivalent was used in lieu of toeboards:  At this commercial building under renovation, the employer failed to provide falling object protection to employees, by not ensuring toe boards were installed on the edges of scaffolds,  to prevent struck-by hazards to employees working below the scaffolds.
