29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Jobsite, 3431 Butler Street, Pittsburgh, PA: On or about 04/08/14 employees were installing siding on the second story balconies of the structure, working from step ladders, and no fall protections systems such as guardrails, safety nets or personal fall arrest were in place to prevent employees from falling 12-15 feet to the ground below.

[59 FR 40738, Aug. 9, 1994; 60 FR 5131, Jan. 26, 1995]: (a) Jobsite, 3431 Butler Street, Pittsburgh, PA: On or about 04/08/14 retraining had not been conducted when employees were observed working without fall protection indicating the employees had not retained the requisite understanding to ensure they understood fall hazards and what methods should be used to prevent falls.
