On or about 09 April 2014, at the above addressed job site; employees were exposed to the hazard of falling while working on a scaffold which was not inspected prior to allowing employees to apply plaster, plywood and wire lard to the side of a building. The scaffold lacked guardrails, cross braces, and was not fully planked.
