29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Worksite, 30 Allen Boulevard, Farmingdale, NY - Employees engaged in roofing activities of a commercial building were standing on the edge of a flat roof approximately 20 feet above ground without being protected by guardrail systems, safety net systems, or a complete fall arrest system; on or about 4/9/14.    NOTE: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement of this violation as normally required by 29 CFR 1903.19.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) Worksite, 30 Allen Boulevard, Farmingdale, NY:  Employees were not provided with fall protection while working on the edge of a  flat roof approximately 20 feet above the ground below. The employees were not trained to recognize the hazards associated with falling; on or about 4/9/14.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Worksite, 30 Allen Boulevard, Farmingdale NY:  Employees used an extension ladder to access a flat roof approximately 20 feet above ground. The ladder was extended to approximately 1 1/2ft above the landing; on or about 4/9/14.    NOTE: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement of this violation as normally required by 29 CFR 1903.19.
