29 CFR 1910.1048(h)(1)(i): All contact of the eyes and skin with liquids containing 1 percent or more formaldehyde was not prevented by the use of chemical protective clothing made of material impervious to formaldehyde and the use of other personal protective equipment such as goggles and face shields, as appropriate to the operation:      a)    QDX Pathology, Cranford, NJ facility, Histology Laboratory: Employees eyes and or face were not protected with personal protective equipment (PPE) such as goggles and face shields when conducting laboratory procedures using 10% Formalin, which contains 4.28% formaldehyde.        Violation occurred on/or about 4/9/14.

29 CFR 1910.1048(i)(3):  Where there was a possibility that an employee's eyes could be splashed with solutions containing 0.1 percent or greater formaldehyde, the employer did not provide acceptable eyewash facilities within the immediate work area for emergency use:        a)    QDX Pathology, Cranford, NJ facility, Histology and UroVysion FISH Laboratories: Employees conducting laboratory procedures using 10% Formalin are exposed to splash to the eyes and/or face to 1% and 4.28% formaldehyde.  The Employer did not provide eyewash facilities in their immediate work area for emergency use.        Reference:  American national Standard for Emergency Eyewash and Shower Equipment (ANSI Standard Z358.1-2004)      Violation occurred on/or about 4/9/14.
