Violation occurred on/or about 4/9/14.

29 CFR 1910.1048(i)(3): Where there was a possibility that an employee's eyes could be splashed with solutions containing 0.1 percent or greater formaldehyde, the employer did not provide acceptable eyewash facilities within the immediate work area for emergency use: a) QDX Pathology, Cranford, NJ facility, Histology and UroVysion FISH Laboratories: Employees conducting laboratory procedures using 10% Formalin are exposed to splash to the eyes and/or face to 1% and 4.28% formaldehyde. The Employer did not provide eyewash facilities in their immediate work area for emergency use. Reference: American national Standard for Emergency Eyewash and Shower Equipment (ANSI Standard Z358.1-2004) Violation occurred on/or about 4/9/14.
