The employer failed to provide certification of corrective action for each cited violation of the OSH Act identified in inspection number 966877.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  On or about 4-09-14, employees who were installing residential second floor decking were walking/working at approximately ten (10) feet above dirt and wooden surfaces without utilizing fall protection equipment.  The Jochman Builders, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA Inspection Number 899242, Citation Number 1, Item Number 1a and was affirmed as a Final Order on 8-29-13, with respect to a workplace located at S Tahoe Lane and S Solitude Lane; Appleton, WI  54915.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a)  On or about 4-09-14, the employer did not provide written certification of employee fall protection safety training for three employees who did not associate fall from elevation hazards when working at heights greater than six feet without utilizing fall protection equipment.  Jochman Builders, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.503(b)(1), which was contained in OSHA Inspection Number 899242, Citation Number 1, Item Number 1b, and was affirmed as a Final Order on 8-29-13 with respect to a workplace located at S Tahoe Lane and S Solitude Lane; Appleton, WI  54915.
