29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   The employer did not provide fall protection to employees who were working near unprotected side or edges at heights above 6 feet above ground. This violation was observed on or about April 09, 2014, at the commercial construction site where employees were exposed to fall hazards while working on the roof of a building at a heights of approximately 21 feet above the ground without a fall protection system.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all employees performing construction activities at heights greater than six feet are protected from falling.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  The employer does not ensure that ladder used by employees is extended 3 feet above the upper landing surface. This violation was observed on or about April 09, 2014, at the construction site where employees were exposed to a 13 foot fall while using an extension ladder to access a roof without extending the ladders side rails at least three feet above the roof landing or securing the top of the ladder to the roof landing.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders used by employees have the ladders side rails extended at least three feet above the roof landing, or securing the top of the ladder to the roof landing.
