29 CFR 1910.137(b)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests in accordance with Tables I-5 and I-6:  a. Where electrical insulating gloves were used during work on electrical equipment and where there was the potential for electrical shock, the gloves were not tested every six months (and according to the testing procedure required by the standard).  Abatement documentation is required for this violation.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures:  a. In the hanging file folder department, at machine P40, pump control and junction boxes were not installed to prevent water from entering the equipment such as by mounting to prevent affecting the equipment and/or by barriers or "weather proofing" the equipment.  (Note: one control pump box was setting loose on the pump table and junction boxes were broken from mounting screws; one junction box was wrapped in tape.)  Abatement documentation is required for this violation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. In the hanging file folder department, such as at the P40 machine, employees cleaned by hand, rotating rollers, allowing for potential contact with moving parts or potential injury from being caught in moving parts.  (Note: industry practice is to prohibit cleaning rotating rollers by direct hand contact.)  Abatement documentation is required for this violation.
