29 CFR 1910.178(l)(2)(ii) as required by 1960.8(b): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. Aleda E. Lutz VA Medical Center, employees required to operate the Taylor-Dunn electric cart, SS-534, S/N 170556 was not provided training on the safe operation of the cart that consisted of practical training, and an evaluation of the operators performance in the workplace.

Lutz VA Medical Center, in the Pipe shop, a power strip was used to power a Magic Chef Microwave, Model # MCD990SC and Rival 12 Cup Coffee Pot, Model # RV-076 which exceeds the recommended 15 amp rating of the power strip exposing employees to an electrical hazard.

29 CFR 1910.178(p)(1) as required by 1960.8(b): Where powered industrial trucks were found to be in need of repair, defective, or in any way unsafe, the trucks were not taken out of service until they had been restored to safe operating condition. Aleda E. Lutz VA Medical Center, in the Pipe shop, the employer did not ensure that a Taylor-Dunn electric cart, Model # SS-534, S/N 170556 was removed from service when found to be in need of repair. Defects on the cart included a non-functioning horn. The Battle Creek VA Medical Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(p)(1), which was contained in OSHA inspection number 531579, citation number 1, item number 2a and was affirmed as a final order on February 11, 2013 with respect to a workplace located at Battle Creek VA Medical Center located at 5500 Armstrong Road, Battle Creek, Michigan 49015.
