Ohio Polymers and Recycling, Inc. - Elyria, Ohio: On or about April 9, 2014, the employer did not assure that employees were protected from falls of approximately 87 inches when working from the platform associated with the densifier.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

After a material delivery, the exit marked "4" in the raw-material area was blocked by recyclable materials that are used in the recycling process. 29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial vehicle:    a. Ohio Polymers and Recycling, Inc. - Elyria, Ohio: On or about April 9, 2014, the employer did not assure that forklift operators had successfully completed a forklift operator safety training program containing the elements in 1910.178(l) before the employee was permitted to operate the forklift.  Employees operate forklifts to lift and transport materials, including bales and boxes of recyclable and recycled plastics. Employees were exposed to injury in that the clamps that secure the propane tank on the truck were defective. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report:  a. Ohio Polymers and Recycling, Inc. - Elyria, Ohio: The employer did not meet the basic requirements for recording injuries and illnesses.  OSHA forms 300, 300A and 301 were not maintained for calendar year 2013.  On or about December 6, 2013, the company experienced a recordable fracture injury. Employees could mistake the doors purpose in that it was marked Exit was also marked Office.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
