29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a.) Independent Concrete Pipe Co. located at 3756 Centennial Rd. Sylvania, OH 43607; On or about April 10, 2014, the employer did not ensure employees were protected on the small Besser pipe machine. Employees are required to perform cleaning activities from the platform on the small Besser pipe machine and are exposed to fall hazards of approximately 9 feet to the concrete below. Independent Concrete Pipe, Co was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(c)(3) which was contained in OSHA inspection number 313787855, citation number 1, item number 2a and was affirmed as a final order on August 5, 2011 with respect to a workplace located at 3756 Centennial Rd. Sylvania, OH 43560.
