General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while preforming window cleaning in elevated places. On or about April 10, 2014, the employer did not ensure that the fall protection ropes and all components of personal fall arrest systems were inspected by a competent person and taken out of service if found to be damaged, exposing employees to a fall hazard of approximately 35 feet. Two employees were attached to a Fitch roof roller supported with one anchor point on a roof without a parapet. The employee on the roof used a 9 foot cut piece of rope to attach to the Fitch roller, rather than an appropriate shock absorbing lanyard with an anchor point separate from the support for the Fitch roller.
