29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    On or about 04/08/14, during an unexpected discharge of hot water and steam from the flash tank near the Congen area, employees were exposed to burns. The employees were exposed in that during the opening of the gat valve of the flash tank, they were not trained or instructed on how to safely remove themselves from the path of flowing hot water and steam, causing burns.
