29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) REAR OF BUILDING - Employees accessed the roof by climbing two extension ladders, and the ladders did not extend the minimum thirty-six (36) inches past the upper landing surface, thereby exposing employees to fall hazards, on or about April 10, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) REAR OF ROOF - Employees performed roofing work on a roof that ranged in pitch from 2/12 to 6/12, and the employees were not using fall protection, thereby exposing employees to fall hazards ranging from approximately thirty (30) feet to forty (40) feet, on or about April 10, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. Russ Wilson Co Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 315884668, citation number one, item number one and was affirmed as a final order on October 7, 2011, with respect to a workplace located at 343 Wildwood Road, Carlisle, PA 17015.
