29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction: At the southeast loading dock, employees were exposed to the hazard of falling into an approximately 21 inch long by 7 inch wide and 16 inch deep hole at the top of a stairway and the provided cover was not constructed to prevent displacement.
