29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work.  The employer does not ensure that work rests are used to support offhand grinding work.  This violation was observed on or about April 10, 2014, in the Spiral Shop area where employees were exposed to caught-in hazards when operating a Dayton bench grinder that did not have a work rest.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that work rests are installed on all grinding equipment used for offhand grinding work.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.     The employer does not ensure that the distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top did not exceed one fourth inch. This violation occurred on or about April 10, 2014, in the Spiral Shop where employees were exposed to struck-by hazards when operating a Dayton bench grinder without a tongue guard and the distance between the abrasive wheel periphery(s) and the safety guard peripheral member at the top exceeded one fourth inch.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that Dayton bench grinder is used so that the distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top does not exceed one fourth inch.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.   The employer does not ensure that unused openings in boxes, cabinets, or fittings are effectively closed. This violation was observed on or about April 10, 2014, in the Welding Shop, in an electrical panel #LN where employees are exposed to electrical hazards when accessing the breakers without the unused openings being effectively closed.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all unused openings  in boxes, cabinets, or fittings are effectively closed.
