On or about April 9, 2014 - at the above addressed jobsite, employees were walking/working around and using electrical equipment/tools near electrical outlets that were not provided with covers or face plates to protect against accidental contact, thereby exposing the employees to electrical hazards.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, shall be equipped with at least one handrail    a. On or about April 9, 2014 - at the above addressed jobsite, the stairway to the second floor deck was not provided with handrails, thereby exposing employees to fall hazards of approximately eleven (11) feet.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     a.  On or about April 9, 2014 - at the above addressed jobsite, employees performing carpentry framing operations from the first floor deck, approximately thirteen (13) feet above the ground were not protected from falls, thereby exposing the employees to fall hazards.     KUCIA CARPENTRY, INCORPORATED WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(13) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314040338, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 1, 2010 WITH RESPECT TO A WORKPLACE LOCATED AT 292 PROSPECT, GLEN ELLYN, IL 60137.
