29 CFR 1904.32(b)(3): The 300A annual summary was not certified by a company executive to ensure that he or she has examined the OSHA 300 Log and that he or she reasonably believes, based on his or her knowledge of the process by which the information was recorded, that the annual summary is correct and complete. (a) Saint Albans, WV location: A company executive did not certify that he or she examined the OSHA-300 log and that the annual summary was correct in that the posted summary for 2013 was certified by the Senior Risk Control Manager, who was not a company executive as defined by 29 CFR 1904.32(b)(4), and the summary numbers did not match the number of injuries on the 2013 log as determined on April 10, 2014. ABATEMENT CERTIFICATION REQUIRED.
