In the grinding area where employees taking their welding tests work, there was an extension cord available for use which did not have a ground pin thereby exposing employees to a contact hazard.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided.  a.     In the A-Pad area where employee were installing rebar for a concrete pad, the employees working on the pad had to climb over a 34 inch wall to access a step ladder thereby exposing employees to a fall hazard.

On the A Pad where employees were installing rebar for a concrete pad, the employees were using a step ladder to climb over the wall to enter the A-Pad thereby exposing employees to a fall hazard.
