Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): On or about April 9, 2014 and times prior thereto, employees of Fike Construction were observed decking the steep sloped rooftop with a pitch of 10:12 for the residence being constructed at or near 5236 Julia Cove in Jonesboro, Arkansas. The employees performing decking were at heights greater than 6 feet from the lower level without fall protection.

On or about April 9, 2014 and times prior thereto, employees of Fike Construction were observed framing a residence being constructed at or near 5236 Julia Cove in Jonesboro, Arkansas. An employee was observed using an A-framed ladder for the purpose for which it was not designed as an unsupported ladder, exposing the employee to fall hazards.
