29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 10, 2014 and at times prior thereto, employees assisting in the installation of underground fuel tanks and vent lines were not wearing hard hats when working at lower levels exposing the employees to head injuries from falling and flying objects.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification.  On or about April 10, 2014, employees installing underground fuel storage tanks and fuel lines were using re-locatable power taps not in accordance with manufacturer and the NFPA 70 listing and labeling instructions to power and operate electrical equipment exposing employees to the hazards of electrical shock and fire hazards:  a) at the trailer, employees were recharging cordless batteries and operating a RIGID 300-T2 115 volt pipe threading machine, and  b) at the pump stations and vent pipes location, employees operated a 10 amp Dewalt reciprocating saw, a 10 amp Dewalt grinder, and a 120 volt, 15 amp Dewalt DW870 chop saw.
