29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:        On or about April 9, 2014 at the above addressed jobsite, employees were using a pneumatic nail gun and were not wearing safety glasses, thereby exposing the employees to the hazards associated with eye injuries.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    On or about April 9, 2014, at the above addressed jobsite, employees performing roofing operations from a ladder jack scaffold, 19.3 feet above the ground, did not utilize a personal fall arrest system, thereby exposing the employees to falls.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about April 9, 2014 at the above addressed jobsite, the portable ladders in the front and back of the house used to access to the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

On or about April 9, 2014 at the above addressed jobsite, employees conducting roofing operations, 12-12.8 feet above the ground, were not protected from falling.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).        The J.B. Roofing & Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10) which was contained in OSHA inspection number 714618, citation number 1, item number 1 and was affirmed as a final order on December 18, 2012, with respect to a workplace located at 321 Roosevelt Road, Glen Ellyn, IL .
