29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Location:  Jobsite, 127 Harrison Avenue, New Canaan, CT  The employee working on a bracket scaffold platform was not protected from a 20 foot fall to the ground by the use of appropriate fall protection systems and/or devices.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  Location:  Jobsite, 127 Harrison Avenue, New Canaan, CT  The employee, required to work from a bracket scaffold to install trim, did not receive appropriate training and information by a competent person to ensure the employee had an understanding of the hazards associated with scaffolds and the steps to take to mitigate those hazards.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  Location:  Jobsite, 127 Harrison Avenue, New Canaan, CT  Employees using an extension ladder to access a scaffold were not given training in all OSHA requirements for the safe use of ladders and the procedures to be followed to minimize and correct those hazards.
