29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:       a) Long ladder from ground to second level:  On or about April 9, 2014, employees were exposed to a fall while ascending or descending a ladder that was not set up properly, the ladder extended approximately one foot above the second level of the roof line.       b) Short ladder from the first level to the second level:  On or about April 9, 2014, employees were exposed to a fall while ascending or descending a ladder that did not extended above the second level roof line.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):        On or about April 9, 2014, employees were exposed to a fall greater than 6 feet  to the ground below while installing and finishing the roof shingles on a roof with multiple pitches of 8:12 and 10:12.  Maria Salas was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 935702, citation number 01, item number 002 and was affirmed as a final order on (12/09/2013), with respect to a workplace located at 10412 Pecan Branch Adkins, TX 78101.
