On 4/8/2014, an employee from Associated Contracting was working on the roof of a three-story Extended Stay America Hotel located at 5555 Veterans Memorial Parkway, St. Peters MO. The employee was observed not wearing eye protection while using a pneumatic nailer.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On 4/8/2014, an employee from Associated Contracting was working on the roof of a three-story Extended Stay America Hotel located at 5555 Veterans Memorial Parkway, St. Peters MO. He was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. The employee was observed on the roof of the building without fall protection, exposing him to a fall to the ground below.
