29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can accidentally walk were not guarded by a floor hole cover of standard strength and construction. a) Jobsite: Warehouse - On or about 4/4/14, employees conducting printing operations were exposed to trip and fall hazards from floor holes missing guards and/or covers. The floor holes measured 12 3/4 inches round, and 17 inches deep.
