OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by metal fragments and/or burned by fire as a result of cylinders being knocked over and leaking, rupturing, exploding, or being propelled:  a) One Plainsboro Road, Plainsboro, NJ 08536 - 3rd Floor Telemetry: Employees were exposed to the hazard of being struck by metal fragments and/or burned by fire as a result of cylinders being knocked over and leaking, rupturing, exploding, or being propelled.  On April 10, 2014 inside patient room 337, a Praxair Grab 'n Go Vantage Type E Oxygen Cylinder, which was not secured from tipping over, was knocked over.  The cylinder ruptured which resulted in an explosion and fire.  An employee working in the room received extensive burns to the body.  The employer did not ensure that the cylinder was secured from falling.     Condition occurred on or about April 10, 2014.
