29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Warehouse, Pick Modular Area: 1) On or about 1-23-14 a worker did not utilize lockout tagout procedures while oiling the chain of a conveyor and injured the index finger of his right hand. 2) On or about 1-23-14 lockout tagout procedures for maintenance work on conveyors had not been developed.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees.: Warehouse, Pick Modular Area: On or about 1-23-14 employees were not trained on the lockout tagout procedures for conveyors. A worker injured his right index finger while oiling a chain drive of a conveyor while it was running.

29 CFR 1910.176(b): Storage of material created a hazard: Warehouse. Employees are exposed to struck by hazards due to unsafe storage: 1) All pallet loads having loose boxes were not shrink wrapped to prevent boxes from falling on workers. 2) A method was not developed to prevent long parts from being stored on higher racks or to be stored behind other long parts to prevent parts from being knocked off storage racks. 3) Loose parts are stored in damaged boxes on pallets on the storage racks.
