On or about April 10, 2014 employees were exposed to contact with live electrical parts because entrances to high voltage transformer closets did not have danger signs warning of high voltage in the following instances:    a) 3rd floor transformer room    b) 6th floor transformer room    c) 9th floor transformer room    d) 12th floor transformer room

29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:    On or about April 10, 2014 employees were exposed to electrical hazards while installing Cat 5 cabling within energized transformer closets on floors 3, 6, 9 and 12. The employees had not been trained to de-energize and lockout energized electrical equipment and circuit parts.
