29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace, including a reasonable estimate of employee exposures to respiratory hazard(s) and an identification of the contaminant's chemical state and physical form:  Plant 1 and 2: Air monitoring had not been conducted to evaluate levels of dust exposure for workers during compounding and Banberry mixing operations.  Air contaminants include but not limited to dusts that may contain silica(talc) and metals.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:  Plant 2;  The Ohio Forge bench grinder was not equipped with adequate tongue guards. The right abrasive wheel did not have a tongue guard and the left wheel had a guard that was bent and not operational.

A black drum of lubrication oil was not labeled with a health hazard warning.  2. Totes of rubber ingredients were not labeled with health hazard warning.  b) Compounding: Totes of rubber ingredients were not labeled with health hazard warnings.  NOTE: Those containers handled by one employee and container contents used within a work shift do not required the hazard warning label.
