29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate. The mezzanine used for storage of merchandize did not indicate the load rating to ensure merchandise stored on steel shelving did not exceeded the capacity of the expanded metal flooring and solid flooring areas.

The NEX's International Bailer Corporation, Model VB-606HD with SN 11158 (cardboard crusher/bailer) was blocked from ready access to it's disconnecting means, in case the emergency stop malfunctions, by garbage cans and brooms/mops.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours. The NEX's was unable to provide copies of the requested OSHA 300 logs for 2013, 2012 and 2011 and 300A's for requested during the opening conference on 4/4/2014 within 4 hours. The safety manager responsible for recordkeeping was not at the facility and the safety investigator did not know how to retrieve the records. The injury illness records were not provided until the next week.
