29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident On or about 19 March 2014, at the Engineering Department, US Coast Guard, La Puntilla, San Juan, Puerto Rico. An oral or written report of a fatality caused by a heart attack at work was not made within 8 hours after the occurrence to the nearest Area Office of the Occupational Safety and Health Administration. NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
