29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) North East Roof: Employees were engaged in roofing activities being exposed to a fall of approximately 30 feet onto an asphalt driveway without being secured with an acceptable means of fall protection, on or about 4/11/14.  b) North West Roof: Employees were engaged in roofing activities being exposed to a fall of approximately 30 feet onto an asphalt driveway without being secured with an acceptable means of fall protection , on or about 4/14/14.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a) North East Roof: Employees that had not been trained in the recognition and procedures to control and minimize fall hazards were were engaged in roofing activities being exposed to a fall of approximately 30 feet onto an asphalt driveway, on or about 4/11/14.
