29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: a) Residential jobsite: On April 11, 2014 and at times prior thereto employees performing roofing activities, which included installing dry sheeting on a 10 in 12 pitched roof, were not properly wearing the fall protection system that was in place. Employees did not have lanyards attached to their full body harnesses. b) Residential jobsite: On April 11 2014 and at times prior thereto an employee performing roofing activities, which included delivering the dry sheet on an 8 in 12 pitched roof, did not have a fall protection system in place. Note) Abatement certification IS required for this item.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: a) Residential jobsite: On April 11, 2014 and at times prior thereto an employee wore a full body harness positioned so that the d-ring was positioned on his back, just above his hips. The employee was performing roof tear off activities on a 10 in 12 pitched roof. Note) Abatement certification IS required for this item.
