29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: The employer does not ensure that each scaffold platform is at least 18 inches wide. This violation was observed on or about April 11, 2014, at the job site where an employee was exposed to a fall hazard when working from a scaffold platform that was not at least 18 inches wide. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees work from scaffold platforms that are at least 18 inches wide.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. The employer does not protect employees on scaffolds from falling when working more than 10 feet above lower levels. This violation was observed on or about April 11, 2014, at the job site where an employee was exposed to a 19 foot fall hazard when working from a scaffold platform without being protected from falling. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when working on scaffolds 10 feet or more above lower levels.
