29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    a. Dairy Cooler and Freezer- On or about April 14, 2014 , employer failed to conduct a workplace hazard assessment to determine the proper selection and type of personal protection needed for it's workers. Thereby exposing employees to environmental temperatures varying forty (40) degree(s) to negative eighteen (-18) degree Fahrenheit   temperatures.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.    a. On or about April 14, 2014, employees were required to add water to forklift batteries that exposed them to the hazard of contact with corrosive acid. Suitable facilities for quick  drenching or flushing of the eyes were not provided within the work area for immediate emergency use.

29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.    a. Level One (1) Cooler- On or about April 14, 2014, pallets of unsalted butter, eggs, mild cheddar, aseptic creamers, whipped cream, and bacon products were being stored upon storage racks with severe column and footplate damage.  This damage was caused primarily from powered industrial vehicles and pallet jack equipment being operated by warehouse workers.  Thereby exposing employees to collapsing materials and struck-by hazard(s).

Throughout Facility- On or about April 14, 2014, employees were required to operate facilities powered industrial vehicles on a daily basis. The employees had not received proper training on the safe operation of these powered industrial vehicles before being assigned to operate them. Thereby exposing employees to struck-by hazard(s).

Safety of equipment shall be determined using electrical insulation;    a. Warehouse near electrical control panel box- On or about April 14, 2014, employees were required to use live electrical charging cables that were severely worn and had significant cuts and abrasions on them. These cuts and abrasions were covered and concealed with black tape. Thereby exposing employees to shock and electrocution hazard(s).

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    a. Warehouse storage area- On or about April 14, 2014, employees had access to electrical junction boxes that had energized wires that were exposed and protruding outside the wall. Thereby exposing employees to shock and electrocution hazard.
