29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:   NAVFAC did not ensure the tongue guard was maintained to within 1/4 inch of the grinding wheel on the Dayton 8" Bench Grinder, Model 4Z909E, lot No. 0107 with NAVFAC Southwest Property Number 6247302221 to prevent debris or broken pieces of the wheel from exiting the grinder housing.
