29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) Construction site, Indian Valley CC club house - On or about April 8, 2014, Employees used a Pettibone 8044 to lift a scaffold platform that was not secured to the forks, to access a roof top work area, exposing them to a fall hazard of more than 10 feet or crushing hazard, if the platform slipped from the forks.
