On 4/9/2014, workers from Redbird Contracting LLC were repairing a roof of a residential building located at 2052 Hannah Drive, Wentzville MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. Four workers were observed on the roof of the building without fall protection, exposing them to fall to the ground below.

On 4/9/2014, workers from Redbird Contracting LLC were repairing a roof of a residential building located at 2052 Hannah Drive, Wentzville MO. They were using a ladder improperly to access the roof. The ladder was not extended to three feet beyond the landing or edge of the roof to which the ladder was used to gain access.
