29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities. a) On or about April 25, 2014, the written standard operating procedure for "Major Ammonia Leak" did not reflect current operating practice as it only instructed for managers and personnel to assemble at the Ualena Street Gate, and did not make reference to choosing an assembly point based upon wind direction. b) On or about April 25, 2014, the written standard operating procedure for "General Procedure for Ammonia Leak" did not reflect current operating practice as it did not contain steps to follow for emergency shutdown of the entire system.
