WORKSHOP: The employer had not established and implemented step-by-step hazardous energy control procedures in the site and machine specifics for the employees during servicing, maintenance and other tasks on equipment, such as (but not limited to) the thermo forming machines, i.e., 150 FM#1.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. WORKSHOP: The employees, as affected and other types were not provided with training and information in the controlling of potentially hazardous and unexpected releases of energy sources (lockout/tagout) during servicing, maintenance and other tasks on equipment, such as (but not limited to) the thermo forming machines, i.e., 150 FM#1. On or about April 02, 2014, an affected employee/operator sustained a finger amputation injury while performing a cleaning task of the Lyle 150 FM No. 1 Thermo Forming Machine.

29 CFR 1904.29(b)(3): You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. ESTABLISHMENT: The following incidents/injuries were not entered on the 2014 OSHA 300 log within seven (7) calendar days: 1. Case 1402; amputation injury which occurred on 04-02-2014 where an operator/employee performing cleaning sustained an finger amputation. 2. Case MV3197648; fracture of left elbow which occurred on 03-18-14 where an employee was returning to the work station and walking between pallets, the employee's right foot got stuck in the pallet and employee fell. ABATED DURING THE INSPECTION.
