The excavation was approximately 60 inches long by 26 inches wide and 8 feet deep and there was no protective system to prevent potential cave-ins. Noted on or about April 14, 2014.

The excavation was approximately 60 inches long by 26 inches wide and 8 feet deep. There was no inspection performed by a competent person prior to the start of the work or during the work for evidence of situations that could result in cave-ins. Noted on or about April 14, 2014.
