29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  In the maintenance shop, an employee cleaned parts with Citgo's 142 Naptha solvent without wearing eye or face protection, exposing the employee to eye irritation and possible long-term damage to the kidneys, lungs, liver, and central nervous system.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion:  In the maintenance shop, the employee cleaned parts with Citgo's 142 Naptha solvent without wearing hand protection, exposing the employee to skin  irritation and possible long-term damage to the kidneys, lungs, liver, and central nervous system.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were not maintained in a legible condition:  In the production area, forklift dataplates were illegible, preventing employees from reading the load limits for the forklifts, exposing employees to a struck by hazard in the following instances:  1)  Caterpiller Forklift, 86-2 had a worn dataplate.  2)  Daewoo Forklift, G2SE, 01-1 had a worn dataplate.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  In the plant, forklift operator training did not consist of practical training and an evaluation of the operator's performance in the workplace before approving them to operate a forklift, exposing employees to a struck by hazard.
