29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:  a) 814 South 19th Street, Philadelphia, PA  Employees were not instructed in the recognition and avoidance of unsafe conditions and the regulations to their work environment to control or eliminate hazards. This condition was observed on or about March 12, 2014.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

This condition was observed on or about March 12, 2014.   Abatement certification and documentation required within 10 days after abatement date. 29 CFR 1926.501(b)(4): Each employee on walking/working surfaces were not protected from falling through holes more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) 814 South 19th Street, Philadelphia, PA  Employees on the walking/working surfaces on a residential construction site were not protected from falling to the next level through holes in the floor. This condition was observed on or about March 12, 2014.

This condition was observed on or about March 12, 2014.
