29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:     This violation occurred on April 16, 2014, at a work site located at 2922 Chatwood Drive, Trophy Club, Texas; employees using pneumatic nail guns were not protected from eye injuries by wearing protective eyewear.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented.     This violation occurred on April 16, 2014, at a work site located at 2922 Chatwood Drive, Trophy Club, Texas; at least three employees, who were walking and working from the roof of a new residential building, were not protected from falling to the lower level by use of guardrail or personal fall arrest system.

29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     This violation occurred on April 16, 2014, at a work site located at 2922 Chatwood Drive, Trophy Club, Texas; employees used ladders that were not extend at least 3 feet above the upper landing to access the working area.
