29 CFR 1926.102(a)(1):     Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.     On or about April 17, 2014, on the roof, the employer did not ensure employees using pneumatic nail guns were protected by the use of protective eyewear.  The employees were actively using pneumatic nail guns on the day of this inspection while installing plywood roof decking on the house.  This condition exposed the employees to serious eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501:     On or about April 17, 2014, on the roof, the employer did not ensure employees installing plywood decking were protected by a fall protection system.  Employees on the north and west areas of the roof were installing plywood decking without any form of fall protection systems in use. This condition exposed employees to a fall of approximately 16 feet to the unfinished ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    On or about April 17, 2014, at the job site, the employer did not ensure that an employee working from an aluminum A-framed ladder was properly using the ladder for the purpose in which it was designed. An employee was installing plywood roof decking on the north side of the roof, while using an aluminum A-framed ladder in the closed position without its metal spreader or locking device engaged to hold the front and back sections in an open position. This condition exposed the employee to the hazard of stumbling and falling from the stepladder during mounting and dismounting.
