29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Roof - On or about 04/04/2014, two carpenters had not been provided with fall protection and were exposed to a 25 foot fall from the top plate of a group of townhomes that were under construction. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) Rear of Building, Ground level - On or about 04/04/2014, carpenters were exposed to a fall while exiting the rear of a building through an opening where there was a 30 inch break in elevation between the buildings first floor and the ground below and no step or ladder was provided.
