At the 4 head resaw, conduit to an outlet and switch was separated. b. The outlet and switch connections at the 4 head resaw were not secured. c. Cover to the conveyor at the resaw area was not secured. d. Cover to the resaw was not secured.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(C) & (D): a. For equipment such as, but not limited to, 4 Head, 5 Head, Single Head band saws, where energy sources included electrical and air, procedures did not include where to specifically lock out the equipment (which disconnect(s) and where located), and how to specifically test the machine to ensure energy isolation. Abatement documentation is required for this violation.
