29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:    On or about 04/17/2014 at the Dollar General Steel Erection Project,  employees were installing "Z" purlins and were not using any fall protection system.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    On or about 04/17/2014 at the Dollar General Steel Erection Project, employees were using an extension ladder to access and descend to and from the upper landing and the ladder did not extend at least 3 feet past the landing.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:    On or about 04/17/2014 at the Dollar General Steel Erection Project, an employee was working from an aerial lift and was not tied off.    The Grenier Construction Company, Inc. of Shrewsbury was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315517953, citation number 01, item number 001b and was affirmed as a final order on 05/02/2012, with respect to a workplace located at Rt 56/Rt 20 in Oxford, MA.
